Here's the gist with karaya gum and Alpelisib (Piqray). Karaya gum is a bulk-forming fiber laxative. When it swells in your gut, it can physically trap or slow down pills that are sitting nearby, which may change how much of a medication actually gets absorbed. With Alpelisib, that could mean a little less drug getting in (less benefit for your breast cancer treatment) or, less commonly, a slight change the other way.

The good news is this concern is theoretical, based on how fiber behaves in general, not on studies with Alpelisib specifically. The simple fix is timing: take your Alpelisib first, then wait about 30 to 60 minutes before the karaya gum. Please run this by your oncology pharmacist or doctor before mixing them.

Karaya gum is a natural plant gum from the Sterculia tree that is mostly used as a bulk-forming laxative and as a thickener or adhesive in food, dental, and ostomy products. As a fiber-type laxative it may help with occasional constipation, but high-quality evidence is limited, and it should be taken with plenty of water to avoid choking or blockage. Talk to your pharmacist or doctor before using it, especially if you have swallowing problems or bowel conditions.

About Alpelisib

What Is Alpelisib Used For?

Brand name: Piqray

Alpelisib is approved for two distinct uses, each tied to a specific brand. Piqray is used in combination with another drug called fulvestrant to treat adults with hormone receptor (HR)-positive, HER2-negative, advanced or metastatic breast cancer whose tumors carry a PIK3CA mutation — and only after the cancer has progressed on or after a hormone-based treatment. A companion diagnostic test is required before starting Piqray.

Vijoice is used to treat adults and children 2 years of age and older who have severe manifestations of PIK3CA-Related Overgrowth Spectrum (PROS) — a rare condition where PIK3CA gene mutations cause abnormal tissue, vascular, and bone overgrowth — and who need systemic (whole-body) therapy. Vijoice's approval for PROS was granted under the FDA's accelerated approval pathway based on response data, and continued approval may depend on confirmatory trials.
