L-cysteine and Palifermin

We checked these two against our interaction database. Here is what we found.

No Known Interaction Found

We did not find a documented interaction between Palifermin and L-cysteine in our sources. This does not guarantee that the combination is completely free of risk — there is often limited research on supplements and herbal products, especially around drug interactions.

If you decide to take both, the practical routine is simple: add one new thing at a time and note the date you started it. If the combination is going to disagree with you, it usually shows in the first days to a couple of weeks — and a start date turns “I’ve been feeling off” into something a pharmacist can actually work with. Do not stop a prescribed medicine on your own.

Always talk with your pharmacist or doctor before making changes to your medications or supplements, particularly if you take several medications or have an ongoing health condition.

How we checked

Why No Interaction Is Listed for Palifermin

Interactions in our licensed database are recorded against drug classes, not just single drug names. Palifermin is filed under a single classification — oral drugs. L-cysteine has documented interactions with 1 drug class — the most serious being antidiabetes drugs (moderate) — and none of Palifermin’s classifications are among them.

Classification data comes from our licensed Natural Medicines dataset. A medication can belong to classes beyond those recorded there, so this describes how the check was run — it is not a safety guarantee.

What to actually watch for

What Palifermin’s Own FDA Label Lists

This is not an interaction — none of it involves L-cysteine. These are the most notable effects listed in the ADVERSE REACTIONS section of Palifermin’s own FDA label, for the medication by itself. Knowing the drug’s own list makes it easier to work out the likely cause if something new turns up while you take both. One of them is worth acting on rather than waiting out — marked below.

  • A higher chance of infection Worth acting on mentioned in 6.2% of FDA reports for Palifermin
  • Fever mentioned in 16.0% of FDA reports for Palifermin
  • Numbness or tingling
  • Joint pain
  • A skin rash mentioned in 10.1% of FDA reports for Palifermin
  • Itching
  • Swelling

From the ADVERSE REACTIONS section of the FDA label for Palifermin. Read the label on DailyMed › Where a percentage is shown, it is the share of 388 voluntary FDA FAERS reports for Palifermin that mention that effect — a reporting proportion, not an incidence rate and not evidence the drug caused it. L-cysteine’s licensed record lists no adverse effects our clinical vocabulary could match, so there is no L-cysteine side to compare — that is a limit of the record, not a statement that L-cysteine has no side effects.

L-cysteine in Pregnancy & Breastfeeding

Pregnancy And Lactation Likely Safe

When used orally in amounts found in foods. There is insufficient reliable information available about the safety of amounts greater than those found in foods; avoid use. Some research suggests that taking L-cysteine during lactation increases levels of free L-cysteine in breastmilk. However, these levels are lower than those found in some commercial hydrolyzed protein infant formulas.

What Palifermin Is Prescribed For, and What L-cysteine Has Been Studied For

These two come up together because they touch the same health topic — not because they do the same job. Palifermin is a prescription medicine with approved uses on its FDA label. L-cysteine is a supplement: what follows is what researchers have looked at it for, and how strong that evidence is.

Palifermin is prescribed for: Kepivance (palifermin) is used to decrease how often and how long severe oral mucositis — painful sores in the mouth and throat — occurs in patients with hematologic malignancies (blood cancers such as lymphoma, leukemia, or multiple myeloma) who are receiving high-dose, cell-damaging (myelotoxic) therapy before an autologous hematopoietic stem cell transplant (a transplant using the patient's own stem cells). Kepivance is specifically indicated when the preparative chemotherapy or radiation regimen is expected to cause severe mucositis in most patients. It is not recommended for use with a high-dose melphalan conditioning regimen, and it was not found to be effective in the setting of allogeneic stem cell transplant (using a donor's cells). Its safety and effectiveness have not been established for patients with non-blood cancers (non-hematologic malignancies).

L-cysteine has been studied for:

  • Cancer studied, not established L-cysteine has been studied for this. Natural Medicines rates the evidence: Insufficient Reliable Evidence To Rate Read what the research shows

    L-cysteine for cancer

Although there is interest in using oral L-cysteine for prevention of cancers of the upper gastrointestinal tract, there is insufficient reliable information about the clinical effects of L-cysteine for this purpose.

Effectiveness ratings and the research summaries above are the verbatim record from the Natural Medicines database, which grades evidence from “Effective” down to “Insufficient Reliable Evidence To Rate”. A rating below the top of that scale means researchers have looked at it — not that it works, and never that it can stand in for a prescribed medicine. Do not stop or replace Palifermin on your own; if you are taking L-cysteine for the same health topic, tell your prescriber, especially if the medicine is being adjusted to a target.

From our pharmacist’s overview on the Palifermin page

“Palifermin (Kepivance) is given by IV to prevent the severe mouth sores that intensive chemotherapy can cause, most often in patients heading into a stem cell transplant, and I read its interaction list with genuine caution because of how fragile those patients already are. All 17 supplements on file act on absorption through the gut,…”

About L-cysteine

What is L-cysteine?

  • Antioxidant support
  • Skin and hair health
  • Liver support
  • Respiratory and mucus thinning
  • General protein/amino acid supplementation

L-cysteine is a sulfur-containing amino acid your body uses to make proteins and the antioxidant glutathione. Most people get enough from food, and supplements are usually well tolerated, but strong evidence supporting specific health benefits from extra L-cysteine is limited. Talk to your pharmacist or doctor before starting it, especially if you are pregnant, breastfeeding, or have a health condition.

Amino acid (not a plant part) · Capsules, tablets, powder; also available as N-acetylcysteine (NAC)

An interaction check asks whether these two react with each other — it does not ask whether L-cysteine is a good idea for someone in your situation. Here is what our L-cysteine record says about it on its own.

L-cysteine: dosing

There is no official standardized dose of L-cysteine established for general supplement use. Products vary widely in strength, and much of the dosing research actually involves NAC rather than plain L-cysteine.The safest approach is to follow the directions on the product label and not exceed the recommended amount. Higher doses have not been well studied and may increase the risk of side effects.

We do not list a known interaction between L-cysteine and Palifermin. That does not guarantee the combination is risk-free, so confirm with your pharmacist or doctor — especially if you take other medications or have a health condition.
Are L-cysteine and Palifermin used for the same thing?
They overlap in purpose, which usually means the real question is whether you need both rather than whether they are safe together. This page lists the shared uses and how strong the evidence is for L-cysteine in each. That is a conversation for your prescriber, especially if Palifermin is being adjusted to a target.
How was the L-cysteine and Palifermin combination checked?
Interactions in our licensed database are recorded against drug classes. Palifermin is filed under 1 classification, and L-cysteine has documented interactions with 1 drug class — none of Palifermin's classifications are among them. A medication can belong to classes beyond those recorded in our dataset, so this describes how the check was run rather than guaranteeing safety.
Does L-cysteine interact with any other medications?
Yes. L-cysteine has 86 documented drug interactions in our database — Palifermin is simply not one of them. If you take anything else, it is worth checking each one; they are all listed on this page.
Does Palifermin interact with any supplements?
Yes. Palifermin has documented interactions with 17 supplements in our database — L-cysteine is not one of them. The Palifermin interaction page linked on this page lists every one, ranked by severity.
