Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Dextromethorphan HBr, Guaifenesin
Dextromethorphan HBr, Guaifenesin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lime interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Your cough medicine has two parts: dextromethorphan (the cough suppressant) and guaifenesin (which loosens mucus). Dextromethorphan is partly broken down by a liver enzyme called CYP3A4. The lab and animal research on file suggests that plant compounds in lime juice can mildly block that enzyme. In theory, that could let a bit more dextromethorphan linger in your body, which might mean more of its usual effects like drowsiness, dizziness, or feeling a little foggy.
That said, this is weak evidence (from rats given concentrated lime juice), not from people. A normal squeeze of lime in your food or drink is very unlikely to cause a problem. If you're using large amounts of concentrated lime products, just mention it to your pharmacist or doctor first.
