Verapamil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lime interacts with.

Animal research shows that phenolics in lime juice inhibit cytochrome P450 3A4. This has been demonstrated in rats treated with sildenafil, a CYP3A4 substrate, and high doses of lime juice.

Here's the gist: your body uses an enzyme called CYP3A4 to break down verapamil. Some lab and animal studies suggest the plant compounds in lime juice can slow that enzyme down. If that happens in people, verapamil could clear more slowly and build up a bit, making its effects stronger than expected. Verapamil isn't a prodrug, so more of it in your system means more action, not less.

In real life that might look like lower blood pressure, dizziness, lightheadedness, a slower heartbeat, or swollen ankles. The good news: this evidence is weak (only animal research, and at high doses of lime juice), so a slice in your water or a squeeze on dinner almost certainly won't matter. Before using concentrated lime supplements or large amounts regularly, just check with your pharmacist or doctor.

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
