Irinotecan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lime interacts with.

Animal research shows that phenolics in lime juice inhibit cytochrome P450 3A4. This has been demonstrated in rats treated with sildenafil, a CYP3A4 substrate, and high doses of lime juice.

Here's the gist for irinotecan (Camptosar). Your body uses an enzyme called CYP3A4 to help process this chemotherapy drug. The interesting wrinkle is that irinotecan is actually a prodrug: it has to be converted into its active form, called SN-38, to fight cancer cells. Certain natural compounds in lime juice have shown the ability to slow down CYP3A4, but so far that's only been seen in lab and animal studies, not in people taking irinotecan.

Because the chemistry here is complicated, blocking CYP3A4 could shift how much active drug you get, possibly affecting side effects like diarrhea, low blood counts, or nausea. A squeeze of lime on your food is very unlikely to matter. Still, with chemo this important, please run any concentrated lime supplements past your oncology team first.
