These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Lime interacts with whole categories of medication. Acetaminophen, Chlorpheniramine Maleate, Dextromethorphan HBr falls into 2 of those categories, so it can interact with Lime in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
PHOTOSENSITIZING DRUGS
1 of 2Why this applies to Acetaminophen, Chlorpheniramine Maleate, Dextromethorphan HBr
Acetaminophen, Chlorpheniramine Maleate, Dextromethorphan HBr falls under PHOTOSENSITIZING DRUGS — the group Lime interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your Vicks Formula 44M (which is a combo of acetaminophen, chlorpheniramine, and dextromethorphan). This particular interaction is about lime oil, not the wedge you squeeze into a drink. Certain compounds in concentrated lime oil can make skin more sensitive to sunlight, and some medicines carry that same tendency. Stacked together, the concern is a slightly higher chance of phototoxicity, basically an exaggerated sunburn or skin reaction on areas that get sun.
I'll be straight with you: this is mostly theoretical, based on lab work rather than real patient cases, so I wouldn't lose sleep over it. Eating limes or drinking lime water is very unlikely to matter. If you're applying concentrated lime essential oil to your skin and heading outside, use sunscreen and run it by your pharmacist first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Acetaminophen, Chlorpheniramine Maleate, Dextromethorphan HBr also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This multi-symptom cold product contains three active ingredients, and one of them, dextromethorphan, is partly broken down by an enzyme called CYP3A4. Lab and animal studies suggest that plant compounds in lime juice may slow that enzyme down a little. In theory, if dextromethorphan is cleared more slowly, you could feel more of its effects, like drowsiness, dizziness, or an upset stomach.
Here's the honest part: this is based only on rat research using high doses of lime juice, not real human data. A squeeze of lime on your food or in a drink is very unlikely to cause any problem. I wouldn't lose sleep over it, but if you plan to use concentrated lime products regularly, just run it by your pharmacist or doctor first.
