Lonafarnib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lime interacts with.

Animal research shows that phenolics in lime juice inhibit cytochrome P450 3A4. This has been demonstrated in rats treated with sildenafil, a CYP3A4 substrate, and high doses of lime juice.

Here's the gist for Zokinvy (lonafarnib). Your body clears this medicine using an enzyme called CYP3A4. Some lab and animal research suggests that plant compounds in lime juice can slow that same enzyme down. If that happened in people, lonafarnib could clear more slowly and build up in your blood, which may mean stronger side effects like nausea, vomiting, diarrhea, poor appetite, or tiredness.

The honest part: this is only theoretical for lonafarnib, based on animal data using high doses of lime juice, not human studies. A squeeze of lime in your cooking or drink is very unlikely to matter. Concentrated lime products are a different story. Since lonafarnib already has strict food and drug rules, please run any lime supplements past your doctor or pharmacist first.

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
