Cyclothiazide is a thiazide-type water pill that helps lower blood pressure and clear extra fluid. The concern here is simple: malic acid, in concentrated supplement form, might give your blood pressure an extra nudge downward on top of what your medication already does. So in theory the two together could lower your pressure a little more than expected.

I want to be honest about how soft this evidence is. This is purely theoretical, based mostly on animal studies, not real-world human reports. The malic acid in an apple or used to flavor food is very unlikely to matter. If you take a supplement dose, just watch for dizziness, lightheadedness when standing up, or feeling unusually tired, and check with your pharmacist or doctor before starting it.

Malic acid is a natural acid found in apples and other fruits, often combined with magnesium in supplements marketed for fibromyalgia and muscle pain. The evidence for its health benefits is weak and limited, and it is mostly used as a food flavoring. It is generally well tolerated in food amounts, but talk to your pharmacist or doctor before using supplement doses.
