Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Oregon Grape interacts with.

Theoretically, Oregon grape might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of Oregon grape, moderately inhibits cytochrome P450 3A4 (CYP3A4).

Here's the gist for Calquence (acalabrutinib). Your body relies on a liver enzyme called CYP3A4 to clear this drug from your system. Oregon grape contains berberine, which lab and early human studies suggest can mildly slow that same enzyme. If it does, acalabrutinib could clear more slowly and build up a bit, which may mean stronger effects and a higher chance of side effects like bruising, bleeding, headache, fatigue, low blood counts, or infections.

I'll be honest: this concern is theoretical, based mostly on how berberine behaves in the lab, not on real cases with this exact drug. Still, since acalabrutinib is a serious cancer medicine, please don't start Oregon grape without first checking with your oncologist or pharmacist.

Here's the basic idea with Oregon grape and your Calquence (acalabrutinib). Your body uses a kind of cellular pump called P-glycoprotein to move acalabrutinib along and help limit how much gets absorbed. Oregon grape contains berberine, which in lab studies seems to slow that pump down. If the pump works less, in theory a bit more acalabrutinib could end up in your bloodstream.

More drug than expected could mean stronger side effects like headaches, bruising or bleeding, low blood counts, or infections. I want to be honest though: this warning comes only from test-tube research, not from real patients, so we don't truly know if it matters in people. Since acalabrutinib is a serious cancer medicine, please run any Oregon grape supplement by your oncologist or pharmacist before starting it.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
