Acetaminophen, Chlorpheniramine, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Oregon Grape interacts with.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of Oregon grape, can inhibit cytochrome P450 2D6 (CYP2D6).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo cold product has one ingredient worth flagging here: dextromethorphan, the cough suppressant. Your body relies on an enzyme called CYP2D6 to clear it. Berberine, the active compound in Oregon grape, may slow that enzyme down a bit. If it does, dextromethorphan could build up and lead to more drowsiness, dizziness, or that foggy, off-balance feeling. The acetaminophen, chlorpheniramine, and pseudoephedrine aren't really driven by this same pathway.
Keep in mind this is mostly theoretical and based on limited evidence, so for occasional short-term use it likely won't cause big problems. Still, if you take Oregon grape regularly or notice extra sleepiness, check in with your pharmacist or doctor before combining them.

In vitro research and preliminary clinical evidence suggest that berberine, a constituent of Oregon grape, moderately inhibits cytochrome P450 3A4 (CYP3A4).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This cold and cough combo contains four ingredients, and one of them, dextromethorphan (the cough suppressant), is partly broken down by the CYP3A4 enzyme. Oregon grape contains berberine, which lab studies suggest can mildly slow that enzyme down. In theory, that could let a bit more dextromethorphan build up, which might add to drowsiness, dizziness, or a foggy feeling, especially since chlorpheniramine in this product is already sedating.
I want to be honest though: this is a theoretical concern based mostly on test-tube data, not real-world reports. For most people taking a short course of a cold medicine, it's unlikely to cause trouble. Still, if you take Oregon grape regularly, it's worth a quick chat with your pharmacist before combining the two.
