Acetaminophen, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Oregon Grape interacts with.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of Oregon grape, can inhibit cytochrome P450 2D6 (CYP2D6).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece to pay attention to in this combo cold medicine: the dextromethorphan (the cough suppressant) is cleared from your body mainly by a liver enzyme called CYP2D6. Oregon grape contains berberine, which lab studies and some early human data suggest can slow that enzyme down. If CYP2D6 is blocked, dextromethorphan can build up higher than usual, which may mean more dizziness, drowsiness, or feeling foggy. The acetaminophen and pseudoephedrine parts aren't really affected this way.
To be fair, this is mostly theoretical and rated moderate, so for an occasional dose it's usually not a big deal. Still, if you take Oregon grape regularly, run it by your pharmacist or doctor before pairing the two.

This combo cold-and-flu product has three active ingredients, and the one that matters here is dextromethorphan, the cough suppressant. Berberine, the main compound in Oregon grape, may gently slow down a liver enzyme called CYP3A4 that helps clear dextromethorphan. In theory, that could let a bit more linger in your system, nudging up side effects like dizziness, drowsiness, or a foggy, lightheaded feeling. The acetaminophen and pseudoephedrine parts aren't really driven by this pathway.
Keep this in perspective. This concern is theoretical, based mostly on lab data, so for a short course of cold medicine it's likely minor for most people. Still, if you take Oregon grape regularly, mention it to your pharmacist or doctor before pairing the two so they can give you a personal thumbs-up.
