Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thinking on this pairing. Abatacept (Orencia) works by calming down an overactive immune system, which is exactly what eases the joint pain, swelling, and stiffness of rheumatoid arthritis. Panax ginseng, on the other hand, may gently stimulate immune activity. So in theory, ginseng could push in the opposite direction and make your abatacept work a little less well, meaning your arthritis symptoms might not stay as well controlled.

I want to be honest about the evidence here: this is mostly theoretical, not something proven in real patients. It's a reasonable caution rather than a strong warning. If you'd like to try ginseng while on Orencia, just run it by your pharmacist or rheumatologist first so they can keep an eye on how you're doing.

From our pharmacist’s overview on the abatacept page

“Abatacept (Orencia) treats rheumatoid arthritis and related conditions by dialing down immune activity, so the supplements that matter here are the ones pulling in the opposite direction. All 53 ingredients on this list share that single theme: herbs and mushrooms with immune-stimulating properties, echinacea, ashwagandha, and astragal…”
