Why this applies to Acalabrutinib

Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the situation with Calquence (acalabrutinib): your body relies on a liver enzyme called CYP3A4 to break it down. Panax ginseng can nudge that enzyme in either direction. If ginseng slows it down, more acalabrutinib could build up, which may mean more side effects like bruising, bleeding, headaches, or infection risk. If ginseng speeds the enzyme up (as it did with a sedative called midazolam in one study), your acalabrutinib could clear faster and work less well against the cancer.

I want to be honest: this is mostly theoretical for acalabrutinib specifically, and the evidence is mixed. But because this is a cancer drug where steady levels matter, I'd play it safe. Please check with your oncologist or pharmacist before adding ginseng.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
