Panax Ginseng interacts with whole categories of medication. Brexpiprazole falls into 2 of those categories, so it can interact with Panax Ginseng in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Panax Ginseng interacts with.
There is some evidence that Panax ginseng can inhibit the CYP2D6 enzyme by approximately 6%. In addition, in animal research, Panax ginseng inhibits the metabolism of dextromethorphan, a drug metabolized by CYP2D6, by a small amount. However, contradictory research suggests Panax ginseng might not inhibit CYP2D6. Until more is known, use Panax ginseng cautiously in patients taking drugs metabolized by these enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your body uses a liver enzyme called CYP2D6 to help clear brexpiprazole (Rexulti). The thought here is that Panax ginseng could slightly slow that enzyme down, which in theory might let a little more brexpiprazole build up in your system. Brexpiprazole is an active medication (not a prodrug), so more of it could mean a bit more of its usual effects, like drowsiness, restlessness, dizziness, or weight changes.
That said, I want to keep this in perspective. The evidence is weak and conflicting. One study suggested ginseng blocks CYP2D6 by only about 6 percent, and other research found no real effect at all. So any change is likely small, if it happens at all. Still, it is worth a quick chat with your pharmacist or doctor before adding ginseng, especially when you start or change a dose.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the honest picture: your body uses an enzyme called CYP3A4 to help break down brexpiprazole (Rexulti), and Panax ginseng can nudge that enzyme in either direction. The data is mixed and mostly theoretical, so I can't tell you for certain which way it would go for you. If ginseng speeds up CYP3A4, your brexpiprazole could clear faster and work less well, meaning symptoms might creep back. If it slows the enzyme, levels could rise, which may bring more drowsiness, restlessness, weight changes, or feeling jittery.
Brexpiprazole is not a prodrug, so it relies on its parent form working directly. Because the effect is unpredictable, please loop in your pharmacist or prescriber before adding ginseng rather than changing anything on your own.

What Is Brexpiprazole Used For?
Brand name: Rexulti
Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.
Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
