Brompheniramine, Dextromethorphan, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase levels of drugs metabolized by CYP2D6. However, research is conflicting.
There is some evidence that Panax ginseng can inhibit the CYP2D6 enzyme by approximately 6%. In addition, in animal research, Panax ginseng inhibits the metabolism of dextromethorphan, a drug metabolized by CYP2D6, by a small amount. However, contradictory research suggests Panax ginseng might not inhibit CYP2D6. Until more is known, use Panax ginseng cautiously in patients taking drugs metabolized by these enzymes.

If you take a combo cough and cold product like Dimetapp DM along with Panax ginseng, here's the gist. The dextromethorphan piece (the cough suppressant) is cleared by a liver enzyme called CYP2D6. Ginseng might block that enzyme a little, so in theory dextromethorphan could hang around longer. If that happened, you might notice more of its usual effects, like drowsiness, dizziness, or feeling foggy.

Honestly, though, the evidence here is shaky. One estimate puts the enzyme slowdown at only about 6 percent, and other studies show no real effect at all. So this is mostly a theoretical concern. It's a good idea to mention you take ginseng to your pharmacist, but for most healthy adults this combo is unlikely to cause trouble.

Here's the gist for Dimetapp DM: the part that matters most with ginseng is the phenylephrine, which is a decongestant that mildly stimulates your system. Panax ginseng can have its own gentle stimulant-like effects. Put them together and, in theory, those effects could add up a bit. You might notice things like a faster or pounding heartbeat, jitteriness, a touch of higher blood pressure, or trouble falling asleep (which the brompheniramine's drowsiness may or may not balance out).

Now, I want to keep this in perspective. This is a theoretical concern based on how these ingredients work, not strong proof. For most healthy people taking a cold product short-term, it's usually mild. Still, if you have high blood pressure or heart issues, check with your pharmacist or doctor before combining them.
