Chlorpheniramine Tannate, Phenylephrine Tannate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the honest picture: this product pairs an antihistamine (chlorpheniramine) with a decongestant (phenylephrine), and the antihistamine piece is partly broken down by a liver enzyme called CYP3A4. Ginseng can nudge that enzyme either direction, so the effect on these medicines is unpredictable rather than dramatic. Neither ingredient is a prodrug, so they don't need the enzyme to become active.

If ginseng slowed clearance, you might notice more chlorpheniramine effects like drowsiness, dry mouth, or dizziness. If it sped clearance up, the cold-symptom relief could feel weaker. Keep in mind this is theoretical and the evidence is limited, so most people won't see a big change. Still, if you take ginseng regularly, mention it to your pharmacist or doctor.

The piece in your Ricobid that matters here is phenylephrine, a decongestant that gently revs up your system (it can raise blood pressure and heart rate). Panax ginseng is thought to have its own mild stimulant-like activity. Put the two together and, in theory, those effects could stack up a bit.

What might that feel like? Things like a racing or pounding heart, jitteriness, higher blood pressure, or extra trouble sleeping, especially since the chlorpheniramine half can already leave some folks feeling oddly wired. To be clear, this is a theoretical concern based on how these substances act, not strong proof, so it is usually not a big deal for healthy people. Still, if you have high blood pressure or heart issues, please run it by your pharmacist or doctor first.
