Why this applies to Elbasvir, Grazoprevir

Elbasvir, Grazoprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the situation with Zepatier (elbasvir and grazoprevir) and Panax ginseng. Your liver uses an enzyme called CYP3A4 to help process this hepatitis C medicine. Ginseng can nudge that enzyme in either direction, so the concern goes two ways. If ginseng slows the enzyme, the drug could build up and you might see more side effects like tiredness, headache, or nausea. If ginseng speeds it up, the drug levels could drop, and lower levels may mean it doesn't clear the virus as well.

To be fair, this is mostly theoretical, based on how ginseng affected other CYP3A4 drugs rather than Zepatier itself. Since finishing a full hepatitis C course matters so much, I'd just run the ginseng by your pharmacist or doctor before combining them.

About Elbasvir and Grazoprevir

What Is Elbasvir and Grazoprevir Used For?

Brand name: Zepatier

Zepatier is used to treat chronic hepatitis C virus (HCV) genotype 1 or genotype 4 infection in adults and children 12 years of age and older (or weighing at least 30 kg). It works against both the most common strain of HCV (genotype 1) and genotype 4.

Depending on the specific genotype, prior treatment history, and certain viral characteristics, Zepatier may be prescribed alone or combined with another antiviral medicine called ribavirin. Your prescriber will determine the right regimen and duration for your situation.
