Here's the concern with combining St. John's wort and Zepatier (elbasvir and grazoprevir). St. John's wort revs up a liver enzyme called CYP3A4, and your body uses that same enzyme to process this hepatitis C medication. When the enzyme speeds up, it clears the drug out faster, which can drop the levels of elbasvir and grazoprevir in your blood.

Why does that matter? Lower drug levels mean the medication may not fully suppress the hepatitis C virus. That raises the real risk of treatment not working and the virus developing resistance, which could limit your future options. This is considered a major, likely interaction, so please don't pair St. John's wort with Zepatier without first talking to your pharmacist or doctor.

What Is Elbasvir and Grazoprevir Used For?

Brand name: Zepatier

Zepatier is used to treat chronic hepatitis C virus (HCV) genotype 1 or genotype 4 infection in adults and children 12 years of age and older (or weighing at least 30 kg). It works against both the most common strain of HCV (genotype 1) and genotype 4.

Depending on the specific genotype, prior treatment history, and certain viral characteristics, Zepatier may be prescribed alone or combined with another antiviral medicine called ribavirin. Your prescriber will determine the right regimen and duration for your situation.
