Why this applies to nefazodone

nefazodone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the gist for nefazodone (Serzone). Your liver uses an enzyme called CYP3A4 to clear nefazodone from your body. Panax ginseng can nudge that enzyme either way, so the honest answer is the direction isn't certain. If ginseng speeds the enzyme up, your nefazodone could clear faster and feel a bit weaker, which might let your depression symptoms creep back. If it slows the enzyme, levels could climb and you might notice more drowsiness, dizziness, dry mouth, or lightheadedness when standing.

Keep in mind this is mostly theoretical, and ginseng can also disturb sleep on its own. Nothing here means you should stop either one. Just loop in your pharmacist or doctor before adding a ginseng supplement so they can keep an eye on how you're doing.

About Nefazodone

What Is Nefazodone Used For?

Brand name: Serzone

Nefazodone Hydrochloride tablets are approved to treat depression, specifically major depressive disorder (MDD). Clinical trials established its effectiveness over 6 to 8 weeks in outpatients and 6 weeks in hospitalized patients with depression.

Nefazodone has also been shown to help prevent depression from coming back in patients who responded well to initial treatment. Because of a serious risk of liver failure, prescribers are advised to consider other antidepressants before turning to nefazodone.
