Pralsetinib falls under QT INTERVAL-PROLONGING DRUGS — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng has an additive effect with drugs that prolong the QT interval and potentially increase the risk of ventricular arrhythmias. However, research is conflicting.
Clinical research shows that short-term use of Panax ginseng can increase the QT interval. However, no changes in QT interval have been identified with prolonged use.

Here's the concern with pairing Panax ginseng and Gavreto (pralsetinib). Pralsetinib can lengthen something called the QT interval, which is a measure of your heart's electrical rhythm. Some short-term studies suggest ginseng may nudge that interval slightly longer too. Stacked together, the worry is an additive effect that, in theory, could raise the risk of an abnormal heart rhythm.

I want to be honest about how soft this evidence is. The research is conflicting, and longer-term ginseng use hasn't shown the same QT change. So for most people this is a theoretical caution, not a sure thing. Still, because you're on a serious cancer medicine, please run any ginseng product past your oncology team or pharmacist before starting it.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Pralsetinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the situation with Gavreto (pralsetinib) and Panax ginseng. Your body relies on a liver enzyme called CYP3A4 to help break down pralsetinib, and ginseng can nudge that enzyme in either direction. That's why this one is hard to predict. If ginseng slows the enzyme down, pralsetinib could build up and bring on more side effects like fatigue, high blood pressure, constipation, mouth sores, or changes in your blood counts. If ginseng speeds the enzyme up, your cancer medicine could clear faster and work less well, which is a real concern with a drug treating lung or thyroid cancer.

Honestly, this is mostly theoretical for pralsetinib and based on limited research, so don't panic. But because this medicine is so important, please run any ginseng product by your oncology pharmacist or doctor first.

What Is Pralsetinib Used For?

Brand name: Gavreto

Gavreto (pralsetinib) is used to treat adults with metastatic RET fusion-positive non-small cell lung cancer (NSCLC) — a type of lung cancer that has spread and carries a specific genetic alteration detected by an FDA-approved test.

Gavreto is also approved for adults and children aged 12 and older with advanced or metastatic RET fusion-positive thyroid cancer who need systemic (whole-body) treatment and whose cancer no longer responds to radioactive iodine (when radioactive iodine is an appropriate option). This thyroid cancer use is under accelerated approval, meaning continued approval depends on results from confirmatory clinical trials.
