Quizartinib falls under QT INTERVAL-PROLONGING DRUGS — the group Panax Ginseng interacts with.
Clinical research shows that short-term use of Panax ginseng can increase the QT interval. However, no changes in QT interval have been identified with prolonged use.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about Vanflyta (quizartinib): one of its most important safety concerns is that it can stretch out the heart's electrical cycle, something we call QT prolongation. In fact, this drug carries a boxed warning about that very risk, and it's usually monitored with regular EKGs during treatment.
Panax ginseng may also nudge the QT interval a little longer, at least with short-term use. Stacking the two together could, in theory, add up and raise the chance of a dangerous heart rhythm. I'll be honest, the ginseng evidence here is mixed and mostly theoretical, and longer-term use didn't seem to cause changes. Still, since quizartinib already walks a fine line with heart rhythm, this isn't a combo to take lightly. Please check with your oncology team or pharmacist before adding ginseng.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Quizartinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Quizartinib (Vanflyta) is a targeted leukemia medicine that your liver processes using an enzyme called CYP3A4. Panax ginseng can nudge that same enzyme, but the tricky part is that studies point in both directions. In some cases ginseng seems to slow CYP3A4 (which could let quizartinib build up), while in others it speeds the enzyme up (which could lower quizartinib levels). This is mostly theoretical for your exact drug, so nobody can predict which way it would go for you.
Why care? Quizartinib carries a serious warning about heart rhythm changes (a longer QT interval), so higher levels are a real concern, while lower levels could mean weaker cancer control. Please don't start ginseng without checking with your oncology team or pharmacist first. With a drug this sensitive, it's better to be cautious.
