Sofosbuvir, Velpatasvir, Voxilaprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the thing with Vosevi (sofosbuvir, velpatasvir, and voxilaprevir): two of its three pieces, velpatasvir and voxilaprevir, are partly handled by the liver enzyme CYP3A4. Panax ginseng can nudge that enzyme either way, so in theory it might raise or lower the levels of these antivirals in your blood. I want to be honest that this is mostly theoretical, based on how ginseng behaved with other CYP3A4 drugs like midazolam and imatinib, not on direct study with Vosevi.

Why it matters: with hepatitis C, you really want steady, reliable drug levels so the full course clears the virus. If ginseng speeds up clearance, you could get weaker effect and risk treatment not fully working. Since a complete cure is the goal, I'd skip the ginseng and check with your pharmacist or prescriber first.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults without cirrhosis (liver scarring) or with compensated cirrhosis (mild liver scarring, Child-Pugh A). It is specifically for people who have already tried an HCV treatment that did not fully work.

Vosevi is approved for adults with HCV genotypes 1, 2, 3, 4, 5, or 6 who were previously treated with a regimen containing an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who were previously treated with sofosbuvir without an NS5A inhibitor. Note: Vosevi has not been shown to offer additional benefit over sofosbuvir/velpatasvir for adults with genotype 1b, 2, 4, 5, or 6 who received sofosbuvir without an NS5A inhibitor.
