Mavacamten falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Peony interacts with.

Theoretically, use of peony may increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on CYP1A2 and CYP3A4. This effect has not been reported in humans.

Here's the gist for Camzyos (mavacamten): your liver uses an enzyme called CYP3A4 to help clear this medicine out of your body. Peony root, in lab studies, seems to gently slow that same enzyme down. If that happened in people, mavacamten could build up a little higher than expected, and that matters because this is a drug with a fairly tight safety window. Too much can weaken the heart's pumping (lower ejection fraction), which may show up as new shortness of breath, swelling, dizziness, or fatigue.

I want to be honest, though: this is only a theoretical concern from test-tube research, not something proven in real patients. Still, given how carefully mavacamten is dosed and monitored, I'd run any peony use past your cardiologist or pharmacist first.

It blocks CYP3A4, the liver enzyme that clears mavacamten, and too much of this drug can weaken the heart's pumping strength, the very problem careful dosing exists to prevent.”
