Why this applies to Clindamycin
Clindamycin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Peony interacts with.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on CYP1A2 and CYP3A4. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea: your body uses a liver enzyme called CYP3A4 to help break down clindamycin. In lab and animal studies, peony appears to slow this enzyme down a little. If that happened in your body, clindamycin might clear out more slowly and build up somewhat, which could mean stronger effects and a higher chance of side effects like nausea, stomach upset, or diarrhea.
I want to be honest about the evidence though. This is theoretical, based only on test-tube and animal work, and it has not been shown in real people taking clindamycin. So I wouldn't lose sleep over it. Still, if you plan to use peony supplements (concentrated doses, not a casual herbal tea) alongside this antibiotic, give your pharmacist or doctor a quick heads-up first.

What Is Clindamycin Used For?

Clindamycin treats serious infections caused by susceptible anaerobic bacteria. It also treats serious infections from susceptible strains of streptococci, pneumococci and staphylococci. It is generally reserved for people allergic to penicillin, or when a penicillin is not appropriate.
Some product labels list specific infection types: lower respiratory tract, skin and skin structure, gynecological, intra-abdominal, bone and joint infections, and septicemia (bloodstream infection). Clindamycin phosphate vaginal cream 2% is used for bacterial vaginosis. Clindamycin should not be used for meningitis.
