Bendamustine Hydrochloride falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Peony interacts with.

Theoretically, use of peony may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on CYP1A2 and CYP3A4. This effect has not been reported in humans.

Here's the idea behind this pairing. Bendamustine (Bendeka) is partly handled by a liver enzyme called CYP1A2. In lab dishes, peony root seems to slow that enzyme down a bit. If that same effect happened in people, your body might clear bendamustine a little slower, letting more of it linger. With a chemotherapy drug like this, that could mean stronger side effects, things like low blood counts, infections, nausea, tiredness, or mouth sores.

I want to be honest about the strength of this, though. The concern comes only from test-tube and animal studies, not from real patients. So it's theoretical, not proven. Still, because bendamustine is powerful chemo, please don't add peony without first checking with your oncologist or pharmacist. They can tell you if it's safe for your situation.

What Is Bendamustine Injection Used For?

Brand name: Bendeka

Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.

For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
