Nirmatrelvir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Peony interacts with.

Theoretically, use of peony may increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on CYP1A2 and CYP3A4. This effect has not been reported in humans.

Here's the gist with Paxlovid (nirmatrelvir plus ritonavir) and peony. Peony has been shown in test-tube studies to weakly slow down CYP3A4, one of the liver's main drug-processing enzymes. Since this enzyme handles nirmatrelvir, the theory is that peony could nudge its levels up a bit, which might mean a little more chance of side effects like an altered or metallic taste, nausea, diarrhea, or muscle aches.

That said, I'd keep this in perspective. This is only laboratory data, not something seen in real people, and peony's effect looks mild. Paxlovid is also a short course, usually five days. Still, ritonavir already interacts with many things, so it's always smart to run any supplement past your pharmacist or doctor before combining.
