Why this applies to Pirfenidone

Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Peony interacts with.

Theoretically, use of peony may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on CYP1A2 and CYP3A4. This effect has not been reported in humans.

Here's the gist: your body clears pirfenidone (Esbriet) mainly through a liver enzyme called CYP1A2. Lab studies suggest peony root may gently slow that same enzyme down. If that happened in real life, pirfenidone could clear a bit more slowly and build up, which might mean more of its usual side effects, things like nausea, stomach upset, tiredness, dizziness, or skin sensitivity to sunlight.

I want to keep this in perspective, though. This concern comes only from test-tube research with a different drug, not from people taking peony and pirfenidone together. So it's more of a theoretical caution than a proven problem. Still, since pirfenidone already needs careful dosing, it's worth mentioning peony to your pharmacist or doctor before you combine them.

Pirfenidone — available as capsules and tablets under the brand name Esbriet, as well as generic pirfenidone products — is indicated for the treatment of idiopathic pulmonary fibrosis (IPF), a chronic, progressive disease in which scar tissue builds up in the lungs and makes breathing increasingly difficult.

Esbriet capsules and tablets, pirfenidone capsules, and pirfenidone tablets are each specifically approved for IPF. No other uses are supported by the available product labels.
