Polatuzumab Vedotin-Piiq falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Peony interacts with.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on CYP1A2 and CYP3A4. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Polivy and peony root. Polivy is an antibody-drug conjugate, and the cancer-killing piece it carries (called MMAE) is partly broken down by a liver enzyme named CYP3A4. Lab studies hint that peony might slow that same enzyme a little. In theory, that could let more of the active drug hang around, which might mean stronger side effects like low blood counts, numbness or tingling in the hands and feet (neuropathy), fatigue, nausea, or a higher infection risk.
I want to be honest about how thin this is. This signal comes only from test-tube and animal work, not from people, so we really don't know if it matters in real life. Since Polivy is serious cancer therapy, please loop in your oncology team or pharmacist before adding peony rather than deciding on your own.

Polatuzumab vedotin-piiq injection is used along with bendamustine (Belrapzo, Treanda) and rituximab (Rituxan) in adults to treat a certain type of non-Hodgkin's lymphoma (NHL; a type of cancer that begins in a type of white blood cells that normally fights infection) that did not improve or improved but returned after treatment with at least two other chemotherapy medications. Polatuzumab vedotin-piiq is in a class of medications called antibody-drug conjugates. It works by killing cancer cells.
