Why Are There 2 Interactions?
Peppermint interacts with whole categories of medication. Acetaminophen, Dextromethorphan, Pseudoephedrine falls into 2 of those categories, so it can interact with Peppermint in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Peppermint interacts with.
Clinical research in healthy volunteers shows that a single dose of peppermint oil 600 mg inhibits CYP3A4 enzymes and increases the AUC of felodipine, a CYP3A4 substrate. However, in vitro research suggests that peppermint oil only inhibits CYP3A4 at very high concentrations.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This is one of those interactions that sounds bigger on paper than it usually is in real life. Concentrated peppermint oil (think capsules around 600 mg, not a peppermint tea or a candy) can mildly slow down an enzyme called CYP3A4. In this combo product, the dextromethorphan (the cough suppressant) leans partly on that pathway, so in theory its levels could nudge up a bit. That might mean more drowsiness, dizziness, or a foggy, lightheaded feeling.
The evidence here is on the weaker side and somewhat theoretical, so I wouldn't lose sleep over it, especially with occasional cold-medicine use. The acetaminophen and pseudoephedrine pieces aren't really affected this way. If you take peppermint oil capsules regularly, just mention it to your pharmacist so they can keep an eye on things.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro and animal research shows that peppermint oil and peppermint leaf inhibit CYP1A2. However, in clinical research, peppermint tea did not significantly affect the metabolism of caffeine, a CYP1A2 substrate. It is possible that the 6-day duration of treatment may have been too short to identify a difference.

Here's the short version: lab and animal studies hint that peppermint can slow down a liver enzyme called CYP1A2, which plays a small role in clearing some medicines. In theory, that could let a drug build up a bit. But when researchers tested real peppermint tea in people, it didn't meaningfully change how the body handled caffeine, another CYP1A2 substrate. So this is mostly a theoretical concern, and it's rated minor.
For this cold and flu combo (acetaminophen, dextromethorphan, pseudoephedrine), none of these are prodrugs that need activating, so I wouldn't expect peppermint to make them dangerously stronger. Enjoying peppermint tea or candy alongside it is very unlikely to cause trouble. If you're using concentrated peppermint oil capsules regularly, just give your pharmacist a quick heads-up.
