CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Acetaminophen, Chlorpheniramine, Phenylpropanolamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Propolis interacts with.
In vitro research shows that propolis extract can inhibit CYP1A2. However, animal research shows that propolis extract does not significantly affect CYP1A2 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP1A2 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: this combo cold product (acetaminophen for pain and fever, chlorpheniramine for sneezing and runny nose, and phenylpropanolamine as a decongestant) is partly processed by a liver enzyme called CYP1A2. In a test tube, propolis can slow that enzyme down, which in theory could let a little more of these medicines build up. That might mean slightly more drowsiness or dry mouth from the antihistamine, or a bit more of that jittery, wired feeling from the decongestant.
But here's the reassuring part: this is only seen in lab dishes. In animal studies, propolis didn't meaningfully change this enzyme, and it has never been reported in people. Normal use is unlikely to cause trouble. Still, if you take concentrated propolis daily, mention it to your pharmacist so they can keep an eye on things.

Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES

In vitro research shows that propolis can inhibit CYP2E1. This effect has not been reported in humans.

Here's the deal with this combo cold and allergy product, which packs acetaminophen (Tylenol), chlorpheniramine, and phenylpropanolamine together. The piece that matters here is the acetaminophen. Your liver uses an enzyme called CYP2E1 to handle a small slice of acetaminophen, and that pathway is what can create a harmful liver byproduct. In test-tube studies, propolis appears to slow down CYP2E1. In theory, blocking that enzyme could actually shift acetaminophen toward a slightly safer breakdown route, but honestly, this has never been shown to matter in real people.
I wouldn't lose sleep over this one. The evidence is lab-only and very weak. The bigger everyday tip is to stay within safe acetaminophen limits regardless. If you take propolis regularly or have any liver concerns, just run it by your pharmacist or doctor.

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies.
