In vitro research shows that propolis extract can inhibit CYP2D6. However, animal research shows that propolis extract does not significantly affect CYP2D6 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2D6 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

This combination cold and cough product has one ingredient worth a quick word: dextromethorphan, the cough suppressant. Your body clears it using an enzyme called CYP2D6. In a test tube, propolis can slow that enzyme down, which in theory could let dextromethorphan build up a bit and cause more drowsiness, dizziness, or stomach upset.
Here's the reassuring part: this was only seen in lab dishes. In animal studies, propolis didn't meaningfully change CYP2D6 activity, likely because so little of it actually reaches the bloodstream. Nothing like this has been reported in real people. Normal propolis use is very unlikely to cause trouble here. Still, if you take concentrated, high-dose propolis, it's worth a quick check with your pharmacist.

Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the piece that matters for this cold-and-cough combo: the acetaminophen in Conar-A is partly handled by a liver enzyme called CYP2E1. In test-tube studies, propolis seems to slow that enzyme down. If that happened in people, it could shift how acetaminophen is processed. The worry isn't the usual side effects so much as the pathway that can make a harmful liver byproduct, so the real takeaway is just to be careful not to overshoot your acetaminophen dose.
