Acetaminophen, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP2D6.
In vitro research shows that propolis extract can inhibit CYP2D6. However, animal research shows that propolis extract does not significantly affect CYP2D6 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2D6 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

This combination cold and flu product has three active ingredients, and the one that matters here is dextromethorphan, the cough suppressant. Dextromethorphan is partly broken down by a liver enzyme called CYP2D6. In test-tube studies, propolis can slow that enzyme down. If that happened in your body, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or a foggy, off feeling.

The good news is that this is very theoretical. It has only been seen in lab dishes, and animal studies did not show a real effect, likely because not much propolis actually reaches the bloodstream. It has never been reported in people. Occasional cold-relief use is unlikely to be a problem, but if you take concentrated propolis daily, just mention it to your pharmacist.

Here's the gist for this cold-and-flu combo (Tylenol Cold and similar products). In a test tube, propolis can slow down a liver enzyme called CYP1A2, and the dextromethorphan (the cough suppressant) in this product is partly handled by that pathway. In theory, blocking it could let dextromethorphan build up a bit, which might mean more drowsiness, dizziness, or a foggy, lightheaded feeling.

That said, I want you to keep this in perspective. This is purely lab-dish data. When propolis was actually given to animals, it did not meaningfully change this enzyme, and nothing like this has been reported in people. Food and normal propolis doses are very unlikely to matter. If you take concentrated propolis often, just mention it to your pharmacist to be safe.

Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. Here's the deal with this combo cold and flu product (which mixes acetaminophen, dextromethorphan, and pseudoephedrine) and propolis. The piece that matters most is the acetaminophen, which is partly handled by a liver enzyme called CYP2E1. In test tube studies, propolis appears to slow that enzyme down. In theory, slowing it could shift how acetaminophen breaks down. I want to be honest with you though: this has only been seen in lab dishes, never in actual people, so the real-world risk looks quite low.

For everyday use, this isn't something I'd lose sleep over. The bigger safety point with these products is simply not doubling up on acetaminophen from multiple sources. If you take propolis regularly or use a lot of acetaminophen, just run it by your pharmacist or doctor first.
