Benzhydrocodone, Acetaminophen falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP2D6.
In vitro research shows that propolis extract can inhibit CYP2D6. However, animal research shows that propolis extract does not significantly affect CYP2D6 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2D6 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the wrinkle with Apadaz: benzhydrocodone is a prodrug. Your body uses an enzyme called CYP2D6 to convert it into hydrocodone, the part that actually relieves pain. So if something blocks that enzyme, you'd expect less pain relief, not more, because less of the active form gets made.

The theory is that concentrated propolis might slow CYP2D6 a bit. But honestly, the evidence here is very thin. It comes only from lab dish studies, and animal research showed no real effect, likely because propolis is poorly absorbed. This has never been reported in people. Normal use is unlikely to cause trouble, but if your pain medicine seems to stop working well, mention the propolis to your pharmacist or doctor.

Here's the honest picture with Apadaz (benzhydrocodone with acetaminophen) and propolis. The concern on file is that propolis might slow down a liver enzyme called CYP1A2. But this isn't actually a major pathway for clearing this particular pain combo, and the worry comes only from test-tube studies. When researchers gave propolis to animals, they didn't see a real effect, likely because the active compounds barely make it into the bloodstream.

So in practice, this is a theoretical interaction, not something that's been seen in people. I wouldn't expect propolis to noticeably change how strong your pain relief is or to bump up opioid side effects like drowsiness, constipation, or slowed breathing. Still, since this is an opioid, run any new supplement past your pharmacist or doctor first.

Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the thing about Apadaz (benzhydrocodone with acetaminophen): the benzhydrocodone part is actually a prodrug. Your body uses an enzyme system to turn it into hydrocodone, the part that actually relieves pain. The lab idea here is that high doses of propolis might slow down CYP3A4, one of the enzymes involved in handling this medication.

If that slowing happened, the drug could linger a bit longer, which in theory might mean stronger opioid effects like extra drowsiness, slowed breathing, constipation, or nausea. But honestly, the evidence is very thin. This comes only from test-tube and animal studies, results were mixed, and it has never been reported in real people. Food or normal supplement amounts are unlikely to matter much. Still, since this is an opioid, it's worth a quick word with your pharmacist or doctor before combining them.

Here's the situation with Apadaz (benzhydrocodone and acetaminophen) and propolis. The piece that matters here is the acetaminophen, which your body partly clears through a liver enzyme called CYP2E1. In test-tube studies, propolis can slow that enzyme down. The thinking is that if CYP2E1 is slowed, slightly more of the drug could hang around. With acetaminophen the bigger concern is the byproduct that can stress the liver, especially at higher doses or with alcohol.

I want to be honest with you though: this is only lab data, and it has not been seen in real people. So I wouldn't lose sleep over an occasional propolis lozenge. If you're taking concentrated propolis supplements regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.

What Is Benzhydrocodone and Acetaminophen Used For?

Brand name: Apadaz

Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.

Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
