Why this applies to Cablivi
Cablivi falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Propolis interacts with.
In vitro research shows that propolis water extract and the propolis constituent, caffeic acid phenethyl ester, can inhibit platelet aggregation. Additionally, evidence from an animal model shows that taking propolis in addition to warfarin decreases INR, suggesting that propolis can decrease the effectiveness of warfarin.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Cablivi (caplacizumab). This medicine is an antibody that blocks von Willebrand factor, so it lowers your blood's ability to form clots. That's exactly what it's meant to do, but it also means bleeding is its main side effect (things like nosebleeds, bruising, bleeding gums, or heavier bleeding).
Propolis may, at least in the test tube, make platelets a little less sticky. Stack that on top of Cablivi and the concern is simply more bleeding risk. I want to be honest though: this is theoretical and based on lab and animal studies, not real patients on Cablivi. Since people on Cablivi are already very closely monitored, please don't add or stop propolis without checking with your hematology team or pharmacist first.

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. Early research suggests it may help with cold sores, mouth sores, and minor wounds, but high-quality evidence is limited for most uses. Caplacizumab-yhdp injection is used to treat acquired thrombotic thrombocytopenic purpura (aTTP; a disorder in which the body attacks itself and causes clots, low amounts of platelets and red blood cells, and could cause other serious complications) in combination with plasma exchange therapy and immunosuppressant medications. Caplacizumab-yhdp is in a class of medications called antithrombotic agents. It works by blocking the action of a certain substance in the body that causes the symptoms of aTTP.
