CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Propolis interacts with.
In vitro research shows that propolis extract can inhibit CYP2D6. However, animal research shows that propolis extract does not significantly affect CYP2D6 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2D6 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo cough and cold medicine. Both of its active ingredients, the antihistamine chlorpheniramine and the cough suppressant dextromethorphan, are processed by a liver enzyme called CYP2D6. In a test tube, propolis can slow that enzyme down. If that happened in your body, these drugs could build up a bit, which might mean more drowsiness, dry mouth, dizziness, or feeling foggy.
That said, this is really a theoretical concern. It's only been seen in lab dishes, and animal studies didn't show a meaningful effect. It has never been reported in people, likely because not much propolis reaches the bloodstream. I wouldn't lose sleep over it, but if you take concentrated propolis often, just mention it to your pharmacist.

Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist with this combo cold and cough product. The dextromethorphan piece is partly handled by a liver enzyme called CYP3A4. In test-tube studies, propolis might slow that enzyme down, which could in theory let dextromethorphan build up a bit and nudge its effects up. With more in your system, you might feel extra drowsy, dizzy, or foggy. The chlorpheniramine antihistamine already causes sleepiness, so the two together could add up.
That said, this is purely a lab-based, theoretical concern. Animal studies didn't show a real effect, and it hasn't been reported in people. Everyday propolis amounts are very unlikely to matter. If you take concentrated propolis often, just run it by your pharmacist to be safe.

Here's the honest picture with this combo cough and cold product (the chlorpheniramine plus dextromethorphan combo). The concern on file is that propolis might block a liver enzyme called CYP2E1. If that happened, drugs cleared by that enzyme could build up a bit and hit you harder. In theory, that could mean extra drowsiness, dizziness, dry mouth, or feeling foggy, since both ingredients here can already make you sleepy.
But here's the important part: this is based only on lab dish (in vitro) studies, and it has never actually been seen in real people. CYP2E1 also isn't the main pathway for these two ingredients, so I'd put this fairly low on the worry list. Neither drug is a prodrug, so propolis wouldn't weaken them. If you take both regularly or feel unusually drowsy, just check with your pharmacist.
