Enasidenib falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Propolis interacts with.
In vitro research shows that propolis extract can inhibit CYP2C19. However, animal research shows that propolis extract does not significantly affect CYP2C19 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2C19 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist with propolis and enasidenib (Idhifa). Enasidenib is partly cleared by a liver enzyme called CYP2C19. In a test tube, propolis can slow that enzyme down. If that happened in your body, enasidenib could build up a bit, which in theory might mean more of its usual side effects like nausea, diarrhea, low blood counts, or rising bilirubin.
The honest part: this is very weak evidence. It only showed up in lab dishes, and animal studies didn't see a real effect, likely because propolis is poorly absorbed. Nothing like this has been reported in actual people. Still, since enasidenib is a serious cancer drug, I'd run any propolis use past your oncologist or pharmacist first.

However, animal research shows that propolis extract does not significantly affect CYP2D6 activity when administered to rats at doses up to 250 mg/kg. Here's the gist for enasidenib (Idhifa): the concern on file is that propolis, in test-tube studies, can slow down a liver enzyme called CYP2D6. Enasidenib is one of the medicines partly handled by that enzyme, and it works as-is (it's not a prodrug that needs activating). So in theory, if propolis blocked CYP2D6, more enasidenib could linger in your body, which might mean stronger effects or more side effects like nausea, low appetite, or changes in your blood counts.
But I want to keep this in perspective. This is theoretical, based only on lab work. When propolis was tested in animals, it didn't meaningfully change CYP2D6, and nothing like this has been reported in people. Food-level exposure is very unlikely to matter; concentrated supplement doses are the bigger question mark. Since enasidenib is a serious cancer medicine, please run any propolis use past your oncology team or pharmacist first.

However, animal research shows that propolis extract does not significantly affect CYP1A2 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP1A2 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

But I want to keep this in perspective. This is only lab data, and animal studies didn't show the same blocking effect, likely because very little propolis actually reaches the liver intact. It has never been reported in people. Since enasidenib is a cancer medicine, please loop in your oncologist or pharmacist before adding any propolis supplement.

It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
