Why this applies to Fostamatinib

Fostamatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist for Fostamatinib (Tavalisse). Your body turns it into its active form, and that active piece is cleared by a liver enzyme called CYP3A4. In a few lab-dish studies, propolis seemed to slow that enzyme down. If that happened in real life, fostamatinib could build up a bit, which might mean more of its usual side effects like diarrhea, high blood pressure, nausea, or changes in liver tests.

The honest takeaway: this is mostly theoretical. Other lab and animal studies showed no real effect, likely because not much propolis actually reaches your bloodstream. No problems have been reported in people. Normal amounts are probably fine, but if you're taking concentrated propolis supplements, just mention it to your pharmacist or doctor.

What Is Fostamatinib Used For?

Brand name: Tavalisse

Tavalisse (fostamatinib) is used to treat thrombocytopenia — dangerously low platelet counts — in adults with chronic immune thrombocytopenia (ITP). It is specifically for people whose platelet counts have not improved enough with at least one earlier ITP treatment.

The goal is to raise platelet counts high enough to reduce the risk of serious bleeding. If platelet counts do not respond sufficiently after 12 weeks, the medication is typically stopped.
