Why this applies to Fruquintinib

Fruquintinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the situation with propolis and your Fruzaqla (fruquintinib). This cancer medication is partly cleared by a liver enzyme called CYP3A4. In a test tube, some propolis extracts appear to slow that enzyme down. If that happened in your body, fruquintinib could build up a bit, which in theory might mean more of its usual side effects like high blood pressure, mouth sores, hand-foot skin reactions, tiredness, or diarrhea.

But here's the honest part: this concern comes only from lab and animal studies, and it has never actually been seen in people. Real-world propolis seems to be poorly absorbed, so it likely never reaches the liver in strong enough amounts to matter. Still, since this is a serious cancer drug, please run any propolis supplement past your oncology team or pharmacist first.

What Is Fruquintinib Used For?

Brand name: Fruzaqla

Fruzaqla (fruquintinib) is used to treat adults with metastatic colorectal cancer (mCRC) — colorectal cancer that has spread to other parts of the body. It is specifically for patients who have already been treated with several standard chemotherapy regimens and other targeted therapies.

Fruzaqla is intended for patients who have previously received fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and — if their tumor is RAS wild-type and it is medically appropriate — an anti-EGFR therapy.
