Propolis and Avatrombopag Maleate

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Propolis interacts with whole categories of medication. Avatrombopag Maleate falls into 2 of those categories, so it can interact with Propolis in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

1 of 2

Why this applies to Avatrombopag Maleate

Avatrombopag Maleate falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP2C9.
In vitro research shows that propolis extract can inhibit CYP2C9. However, animal research shows that propolis extract does not significantly affect CYP2C9 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2C9 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist for folks taking Doptelet (avatrombopag) alongside a propolis supplement. Your body partly breaks down avatrombopag using a liver enzyme called CYP2C9. In the test tube, propolis can slow that enzyme down. If that happened in a real person, more of the drug could linger in your system, which in theory might nudge your platelet count higher than intended and raise the small risk of a clot.

The important part: this is theoretical. It only showed up in lab studies, and animal research found no meaningful effect, likely because propolis isn't well absorbed. It has never been reported in people, so I wouldn't lose sleep over it. Still, since avatrombopag works on your platelets, it's worth a quick word with your pharmacist or doctor before adding concentrated propolis.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Avatrombopag Maleate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist with propolis and Doptelet (avatrombopag). Your body clears avatrombopag partly through a liver enzyme called CYP3A4. Some test-tube studies suggest propolis might slow that enzyme down, which in theory could let avatrombopag build up a little more than expected. Since avatrombopag's whole job is to raise your platelet count, higher levels could, in theory, push platelets up more than intended.

That said, I want to be honest about how shaky this is. The concern comes only from lab dishes, and animal studies showed no real effect. It has never been reported in actual people, and normal propolis use is unlikely to matter. Still, it's worth a quick mention to your pharmacist or doctor, especially if you're using concentrated propolis supplements.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Avatrombopag Maleate page

“Avatrombopag maleate (Doptelet) exists to raise a low platelet count, so anything that drags its blood level down invites the bleeding risk it was prescribed to prevent. I would handle this list of 54 supplements with real care, and the two rated major come first.”

  • Cold sores and herpes lesions (topical)
  • Sore throat and mouth sores
  • Wound and skin healing
  • Immune support
  • Oral and gum health

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. Early research suggests it may help with cold sores, mouth sores, and minor wounds, but high-quality evidence is limited for most uses. It is generally well tolerated, but allergic reactions can occur, especially in people allergic to bee products.
