Why this applies to Nelfinavir Mesylate

Nelfinavir Mesylate falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP2C19.
In vitro research shows that propolis extract can inhibit CYP2C19. However, animal research shows that propolis extract does not significantly affect CYP2C19 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2C19 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist: your body uses a liver enzyme called CYP2C19 to help break down nelfinavir (Viracept). In test-tube studies, propolis can slow that enzyme down. If that happened in real life, nelfinavir could build up a bit, which might mean more of its usual side effects like diarrhea, nausea, gas, or stomach upset. Nelfinavir is an active drug (not a prodrug), so blocking its breakdown would tend to raise, not lower, its levels.

The important part: this is only a theory from lab work. In animal studies, propolis didn't meaningfully change this enzyme, and it has never been reported as a problem in people. Everyday amounts are unlikely to matter, but concentrated supplements are less certain. Since keeping HIV treatment steady is important, just run it by your pharmacist or doctor before adding propolis.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. Here's the short version on combining propolis with nelfinavir (Viracept). Your body relies on a liver enzyme called CYP3A4 to break nelfinavir down. In a test tube, some propolis compounds appear to slow that enzyme, which could in theory let nelfinavir build up a bit higher than usual. With this HIV medicine, more drug in your blood might mean more of its typical side effects, like diarrhea, nausea, gas, or stomach upset.

That said, I want to be honest about the strength of this. The concern comes only from lab and animal studies, and even in rats propolis didn't meaningfully change this enzyme. It has never been reported in real people. Food-level exposure is very unlikely to matter, but if you're using concentrated propolis supplements, just run it by your pharmacist or HIV doctor first.
