Why this applies to Nirogacestat

Nirogacestat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the short version on pairing propolis with Ogsiveo (nirogacestat). Nirogacestat is cleared from your body mainly by a liver enzyme called CYP3A4. In test-tube studies, some propolis compounds appear to slow that enzyme down, and if that happened in real life, more nirogacestat could linger in your bloodstream. That could mean stronger side effects like diarrhea, nausea, mouth sores, rash, or fatigue.

The reassuring part: this is very theoretical. The lab data conflict with each other, animal studies showed no real effect, and it has never been reported in a person. Everyday amounts of propolis are unlikely to matter, though concentrated supplements are less certain. Since nirogacestat is a serious cancer medicine, just give your oncology pharmacist a heads-up before starting propolis.

Ogsiveo (nirogacestat) is used to treat adults with desmoid tumors that are growing or progressing and require systemic (whole-body) treatment. Desmoid tumors are rare, locally aggressive growths that arise from connective tissue.

This indication is specific to Ogsiveo tablets taken by mouth. It is not approved for use in children, as safety and effectiveness have not been established in pediatric patients.
