Why this applies to Osilodrostat Phosphate

Osilodrostat Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

If you take Isturisa (osilodrostat) for Cushing's disease and you're curious about propolis, here's the honest picture. Osilodrostat is partly cleared by a liver enzyme called CYP3A4. In a few test-tube studies, propolis appeared to slow that enzyme down, which in theory could let osilodrostat build up a bit and push its effects harder (think more fatigue, headache, nausea, swelling, low cortisol symptoms, or changes in your heart's QT rhythm).

But the evidence here is very weak. Other lab studies showed no effect, and animal studies found nothing meaningful, likely because little propolis actually reaches the bloodstream. This has never been reported in real people. Normal amounts probably won't cause trouble, but since osilodrostat is carefully dose-adjusted, check with your doctor or pharmacist before adding concentrated propolis.
