Why this applies to Simeprevir Sodium

Simeprevir Sodium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the deal with combining propolis and simeprevir (Olysio), your hepatitis C medication. Your liver uses an enzyme called CYP3A4 to break down simeprevir. In a test tube, some propolis compounds appear to slow that enzyme down, which could, in theory, let simeprevir build up to higher levels and increase side effects like itching, rash, sun sensitivity, and nausea.

That said, I want to be honest about the evidence: this is based only on lab and animal studies, and it has never actually been reported in people. Animal research even suggests propolis doesn't meaningfully affect this enzyme in the body. So the real-world risk seems low, especially at typical amounts. Still, since simeprevir is a serious antiviral, it's worth a quick check-in with your pharmacist or doctor before adding propolis.

  • Simeprevir is used along with ribavirin (Copegus, Rebetol) and peginterferon alfa (Pegasys) to treat chronic hepatitis C (an ongoing viral infection that damages the liver). Simeprevir is in a class of medications called protease inhibitors. It works by decreasing the amount of hepatitis C virus (HCV) in the body. Simeprevir may not prevent the spread of hepatitis C to other people.
