Here's the simple version. Once your liver converts adrafinil into modafinil, an enzyme called CYP3A4 helps clear that modafinil out of your body. Simeprevir (Olysio) can slow that same enzyme down. In theory, that means modafinil could hang around longer and feel a bit stronger, so you might notice more of its stimulant effects like trouble sleeping, jitteriness, a racing heart, headache, or anxiety.

I'll be honest with you though, this is a theoretical concern based on how the drugs work, not on real patient reports. What worries me more is that adrafinil is unregulated and hard on the liver, and you're treating hepatitis C, which already stresses your liver. Please loop in your doctor or pharmacist before mixing these.

Here's the gist for Olysio (simeprevir). Your body turns adrafinil into modafinil, and modafinil can gently speed up a liver enzyme called CYP3A4. Simeprevir is cleared by that same enzyme. So in theory, taking adrafinil could nudge your body into breaking down simeprevir a little faster, leaving less of it in your blood.

Why that matters: simeprevir works by lowering the amount of hepatitis C virus in your body. If its levels dip too low, the treatment may not fight the virus as well. I want to be honest though, this is a theoretical concern based on how the drugs work, not from strong studies, and the enzyme effect is weak. Still, hep C therapy is too important to gamble with, so please run this by your pharmacist or doctor before combining them.

What Is Simeprevir Used For?

Brand name: Olysio

  • Simeprevir is used along with ribavirin (Copegus, Rebetol) and peginterferon alfa (Pegasys) to treat chronic hepatitis C (an ongoing viral infection that damages the liver). Simeprevir is in a class of medications called protease inhibitors. It works by decreasing the amount of hepatitis C virus (HCV) in the body. Simeprevir may not prevent the spread of hepatitis C to other people.
