Belumosudil Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Syrian Rue interacts with.

Theoretically, Syrian rue might increase the levels and clinical effects of drugs metabolized by CYP3A4.
Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.

Here's the situation with Rezurock (belumosudil) and Syrian Rue. Your body relies on a liver enzyme called CYP3A4 to break down belumosudil and clear it out. Lab studies suggest the harmala alkaloids in Syrian Rue (like harmine) can block that same enzyme. If that blocking happens in people, belumosudil could build up to higher levels than expected.

More drug in your system can mean a greater chance of its side effects, things like nausea, diarrhea, fatigue, swelling, or liver enzyme changes. I want to be honest with you though: this is a theoretical concern based on test-tube research, not real patient cases, so we don't know how strong it actually is. Since belumosudil treats a serious condition, please don't change anything on your own. Run this by your transplant team or pharmacist first.

Rezurock (belumosudil) is used to treat chronic graft-versus-host disease (chronic GVHD) — a serious immune condition that can occur after a stem cell or bone marrow transplant, where the donor's immune cells attack the recipient's body. It is specifically for patients aged 12 and older whose chronic GVHD has not responded to at least two prior systemic (whole-body) treatments.
