Theoretically, Syrian rue might increase the risk of serotonergic adverse effects, including serotonin syndrome, when taken with serotonergic drugs.
Syrian rue contains harmaline and harmine, which have demonstrated strong, reversible inhibition of monoamine oxidase (MAO) A and weak inhibition of MAO B in animal and in vitro research. This may increase the risk for adverse effects with serotonergic drugs.

This combination product mixes an antihistamine (chlorpheniramine), a drying agent (methscopolamine), and a decongestant (phenylephrine). The piece that matters here is phenylephrine, which raises blood pressure by tightening blood vessels. Syrian Rue's harmala alkaloids act like an MAO inhibitor, and MAOIs can amplify the effects of decongestants like this. The concern is a spike in blood pressure, headache, pounding heartbeat, or feeling jittery and flushed.

To be fair, this warning is theoretical and comes mainly from lab and animal studies, not strong human data. Even so, mixing an MAOI-type substance with a decongestant is a well-known caution, so I would not pair them on your own. Please run it by your pharmacist or doctor first.

Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.

This combination product mixes an antihistamine (chlorpheniramine), a drying agent (methscopolamine), and a decongestant (phenylephrine). Lab studies hint that compounds in Syrian Rue can slow down the CYP3A4 enzyme that helps clear some of these medicines. In theory, that could let the drug levels creep up and make their usual effects stronger.

What might that feel like? More drowsiness, dry mouth, blurry vision, constipation, trouble urinating, a faster heartbeat, or feeling jittery. Keep in mind this concern is theoretical, drawn from test-tube research, not real patient cases, so it may not amount to much in practice. Still, Syrian Rue carries its own toxicity risks, so please check with your pharmacist or doctor before pairing the two.

This combo product leans on two ingredients that work by blocking acetylcholine: the antihistamine chlorpheniramine and the drying agent methscopolamine. They calm runny noses and watery eyes partly by dialing down that chemical messenger. Syrian Rue, on the other hand, contains harmine, which in animal studies seemed to raise acetylcholine activity. In theory, that could push back against the drying effect and make your medicine feel a little less effective.

I want to be honest: this is a theoretical concern based on lab and animal data, not real human cases, so I wouldn't lose sleep over it. Still, Syrian Rue is risky on its own (it acts as an MAOI), so please check with your pharmacist or doctor before mixing the two.

Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment. Talk to a pharmacist or doctor before considering it.
