Syrian rue contains harmaline and harmine, which have demonstrated strong, reversible inhibition of monoamine oxidase (MAO) A and weak inhibition of MAO B in animal and in vitro research. This may increase the risk for adverse effects with serotonergic drugs.

Here's the gist with this combo. Syrian Rue's harmala alkaloids act like an MAO inhibitor, meaning they slow down an enzyme that normally clears out serotonin. The dextromethorphan in your Vicks Formula 44D nudges serotonin levels up too, so stacking the two could push serotonin too high. That can lead to serotonin syndrome, with symptoms like agitation, a racing heart, sweating, shivering, muscle twitching, or confusion. On top of that, the phenylephrine is a decongestant that can raise blood pressure, and MAO inhibitors are known to spike blood pressure further.

To be fair, this is mostly theoretical, based on lab and animal studies, not strong human data. Still, given the real risks, please check with your pharmacist or doctor before pairing these.

Here's the thing with this combo: the dextromethorphan in Vicks Formula 44D is cleared from your body by a liver enzyme called CYP2D6. Lab studies suggest the harmala alkaloids in Syrian Rue can block that enzyme, so dextromethorphan could build up higher than usual. That might mean more drowsiness, dizziness, confusion, or feeling lightheaded.

There's a bigger worry too. Syrian Rue acts like an MAO inhibitor, and combining MAOIs with dextromethorphan is a classic recipe for serotonin syndrome (agitation, racing heart, sweating, tremor, high temperature). This is mostly theoretical and based on how these compounds behave, not solid human studies, but I'd treat it seriously. Please check with your pharmacist or doctor before mixing the two.

If that enzyme is partly blocked, dextromethorphan could build up and hit harder, meaning more dizziness, drowsiness, or feeling foggy than you'd expect.

There's a bigger worry, though. Syrian Rue acts as an MAO inhibitor, and mixing MAOIs with dextromethorphan can trigger dangerous serotonin syndrome (agitation, racing heart, high blood pressure, sweating).
