Fruquintinib falls under HEPATOTOXIC DRUGS — the group Syrian Rue interacts with.
Cases of hepatotoxicity and hepatic failure have been reported in patients who ingested a large quantity (up to 100 grams) of Syrian rue seeds.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing Syrian Rue and Fruquintinib (Fruzaqla). This isn't about one changing how the other is broken down. Instead, both can be tough on your liver. Fruquintinib, a targeted cancer medicine, already carries a known risk of raising liver enzymes and causing liver injury, which is why your care team checks bloodwork regularly. Syrian Rue, especially in large seed amounts, has been linked to serious liver problems too.
Stacking two liver stressors could add up. Keep in mind this is a theoretical concern based on how these act, not proven cases together, and the reported harm came from swallowing huge amounts of seeds. Still, I'd skip Syrian Rue while on Fruquintinib and run it by your oncologist or pharmacist first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.

Here's the situation with Fruquintinib (Fruzaqla), the targeted cancer pill used for certain advanced colorectal cancers. Your body clears it partly through a liver enzyme called CYP3A4. Syrian Rue contains compounds (harmine and related alkaloids) that, in lab studies, appear to slow that same enzyme down.
If that slowing happens in real life, Fruquintinib could build up higher than intended, which may worsen its known side effects: high blood pressure, mouth sores, hand and foot skin reactions, fatigue, and bleeding or bruising. I want to be honest, though: this warning is theoretical, based on test-tube data, not proven in people. Even so, since Fruquintinib is a serious cancer drug, please don't pair it with Syrian Rue without first checking with your oncologist or pharmacist.

What Is Fruquintinib Used For?
Brand name: Fruzaqla
Fruzaqla (fruquintinib) is used to treat adults with metastatic colorectal cancer (mCRC) — colorectal cancer that has spread to other parts of the body. It is specifically for patients who have already been treated with several standard chemotherapy regimens and other targeted therapies.
Fruzaqla is intended for patients who have previously received fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and — if their tumor is RAS wild-type and it is medically appropriate — an anti-EGFR therapy.
