Theoretically, Syrian rue might increase the risk of serotonergic adverse effects, including serotonin syndrome, when taken with serotonergic drugs.
Syrian rue contains harmaline and harmine, which have demonstrated strong, reversible inhibition of monoamine oxidase (MAO) A and weak inhibition of MAO B in animal and in vitro research. This may increase the risk for adverse effects with serotonergic drugs.

Here's the concern with pairing Syrian Rue and Tussionex. The harmala alkaloids in Syrian Rue act like a natural MAO inhibitor, meaning they slow down an enzyme that normally clears out serotonin. Hydrocodone, the opioid part of this cough medicine, has mild serotonin-boosting activity of its own. Stacking the two could, in theory, push serotonin too high and trigger what we call serotonin syndrome (agitation, sweating, a racing heart, shivering, muscle twitching, or confusion).

I'll be honest with you: this warning rests on lab and animal research, not solid human studies, so the real-world risk is uncertain. Still, MAO inhibitors mixed with opioids is a combination we take seriously. Please check with your pharmacist or doctor before using these together.

Syrian rue contains beta-carboline alkaloids, including harmine, harmaline, harmalol, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids can inhibit CYP2D6 enzymes.

Here's the wrinkle with Tussionex: the hydrocodone part is actually a prodrug that your body partly converts through an enzyme called CYP2D6 into hydromorphone, a stronger pain reliever. Lab studies suggest Syrian Rue's harmala compounds can block CYP2D6. In theory, blocking that enzyme could shift how much active drug you get. The direction isn't perfectly clean here, because hydrocodone still has pain-relieving effects on its own, but a real concern is that beta-carbolines may also stall the enzyme that clears the hydrocodone itself, letting it build up.

Stacked on top of that, Syrian Rue acts like an MAO inhibitor, and combining MAOIs with opioids can be genuinely dangerous. With an opioid antihistamine combo, watch for heavy sedation, slow or shallow breathing, and confusion. This is theoretical, but please clear it with your pharmacist or doctor first.

Here's the situation with Tussionex (hydrocodone and chlorpheniramine) and Syrian Rue. Lab studies suggest the harmala alkaloids in Syrian Rue can slow down CYP3A4, one of the liver enzymes that helps clear hydrocodone. If that enzyme is blocked, hydrocodone could build up in your body and hit harder than expected.

For an opioid cough product, stronger effects can mean more drowsiness, slowed or shallow breathing, confusion, constipation, and low blood pressure. The chlorpheniramine adds its own sedation on top. Keep in mind this warning is theoretical, based mostly on test-tube data, not real patient cases. Still, because slowed breathing is a serious risk with opioids, please run this combination by your pharmacist or doctor before mixing them.

Syrian rue contains harmine. Animal research suggests that intravenous administration of harmine potentiates the effects of acetylcholine or increases acetylcholine levels.

Tussionex pairs hydrocodone (an opioid cough suppressant) with chlorpheniramine, an antihistamine that also has anticholinergic activity (the part that dries you out and helps calm a cough). The theory here is that Syrian Rue's harmala alkaloids may boost acetylcholine, which is the very chemical anticholinergics work to block. So on paper, Syrian Rue could blunt the chlorpheniramine side of this combo, making it work a little less.

Honestly, this comes only from animal lab data, so it's very theoretical and unproven in people. I'd worry more about a different problem: Syrian Rue acts like an MAO inhibitor, and combining MAOIs with opioids can be dangerous. Please don't mix these without talking to your pharmacist or doctor first.
