Vorapaxar falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Syrian Rue interacts with.

Theoretically, Syrian rue might increase the levels and clinical effects of drugs metabolized by CYP3A4.
Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.

Here's the gist for Vorapaxar (Zontivity): your body relies on a liver enzyme called CYP3A4 to clear it out. Lab studies suggest the harmala alkaloids in Syrian Rue can slow that enzyme down. If the enzyme works slower, Vorapaxar could build up to higher levels than expected in your blood.

Since Vorapaxar already thins your blood by keeping platelets from clumping, more drug in your system could mean a higher bleeding risk, things like easy bruising, nosebleeds, or bleeding that's slow to stop. I'll be honest, this is a theoretical concern from test-tube research, not proven in people, so don't panic. Still, with a blood thinner I'd play it safe. Please check with your pharmacist or doctor before pairing these two.

What Is Vorapaxar Used For?

Brand name: Zontivity

Zontivity (vorapaxar) is used to lower the risk of heart attack, stroke, cardiovascular death, and the need for urgent procedures to open blocked arteries. It is prescribed for people who have already had a heart attack or who have peripheral arterial disease (PAD) — a condition where arteries outside the heart become narrowed or blocked.

Zontivity is not used alone. It is given alongside aspirin and/or clopidogrel as part of a broader antiplatelet treatment plan.
